Madison grew up in Emporia, Kansas, where gymnastics and softball first introduced her to chiropractic — and where she noticed adjustments helped with more than sports. She spent three years as an associate in Sun Prairie before opening Revive.

Her patients include mothers she has cared for through pregnancy and after, and children she has adjusted from three days old.

Born in Madison and raised in Sheboygan, Dr. Sam competed on the NCAA women's bowling team at Marian University while earning her Exercise Science degree — an athlete's understanding of movement she brings into every visit.

She began her career in Rockford, Illinois as one of the team chiropractors for the Rockford IceHogs, the AHL affiliate of the Chicago Blackhawks.

Hailey is the first person you will meet. A competitive swimmer for eight years, a shoulder injury in high school pointed her toward health care — and she has been a chiropractic assistant ever since.

She spent four years assisting in Sun Prairie before joining Revive in 2022.

Before your first visit

Questions we get asked a lot

Do I need a referral to be seen?
No. You can contact us directly and book your first visit yourself. If you would like us to coordinate with another provider, just say so when you write.
Is the adjustment going to be forceful?
Not unless that is what suits you. We keep ten different techniques on hand, including the ProAdjuster360 instrument, specifically so that patients who are nervous about manual adjusting have a gentle option.
Do you see children?
Yes. Dr. Madison has adjusted patients from three days old, and our reviews include families who bring in babies, toddlers and teens.
What if I am pregnant?
Pregnancy and postpartum care is part of what we do. Dr. Madison has cared for patients through both of their pregnancies and beyond. Write to us and describe where you are and we will explain what a visit looks like.
I am an athlete — can you help with performance, not just pain?
Yes. Dr. Sam served as one of the team chiropractors for the Rockford IceHogs and focuses on injury prevention, recovery and performance care alongside general practice.
